Students participating on junior high or varsity teams must have the following forms completed and turned into the head coach before participating in practices or playing in games.
  • OHSAA Physical Form
  • Riding Consent Form

    Fill this form out and return it to the coach before the beginning of the season. 

  • Homeschool K-8 Athletics/ K-12 Drama Admissions

    The K-8 home school athletic/ K-12 drama admissions form should be completed once a year per homeschool family (this only applies to homeschool families that are only involved in athletics or drama. Home school families that take classes will complete this information in FACTS admissions). These families should also download the emergency medical form and turn in or email to the CCS office.
